WelcomeCommunity Partnership for Older Adults is a national program of the Robert Wood Johnson Foundation. The goal of the program is to foster community partnerships to improve long term care and supportive services systems to meet the current and future needs of older adults. The program’s guiding principles: - Long term care begins at home and in the community with individuals and their families.
. - Communities and older adults working together are the key to developing long term care solutions for tomorrow.
. - Community Partnerships build bridges between the long term care options of today and of the future.
. - By learning locally and sharing nationally, community partnerships develop solutions for long term care.
Milwaukee’s Connecting Caring Communities is part of the Community Partnership for Older Adults program. We are one of eight grantees nationally to receive an implementation grant from the foundation.
