Why should my organization or company get involved with Connecting Caring Communities?
Connecting Caring Communities is committed to making Milwaukee County a place where people of all ages are able to live in the setting they choose. Providing options for those who need long term care is critical to this goal. Most of us will eventually need help from others, and knowing how to access that help will make decision making easier. If you are a long term care service provider, or know someone who might have long term care need, makes you a stakeholder. Our vision is for all stakeholders to be participants in Connecting Caring Communities.
How can I get involved with Connecting Caring Communities?
Visit the contact us page and send us an e-mail, or call any of the contact persons listed on the various project pages. We are continuously looking for new partners and new opportunities to improve the long term care system for Milwaukee’s older adults.
How does Connecting Caring Communities define long-term care?
Long term care begins when an individual can no longer do an activity that they had done in the past. This may start with someone losing the physical ability to shovel snow or mow the lawn. But long term care also includes all areas of our lives; planning for long term care involves making decisions and plans for your spiritual well-being, medical needs, social activities, other live issues.
Does Connecting Caring Communities target a specific age group?
Most long term care recipients are older adults, and their caregivers are, more often than not, family members.
We are targeting older adults and their families; particularly family members who are actively involved in the decision making processes with the older adult.
When will Connecting Caring Communities end?
The Robert Wood Johnson funding is a four year grant, the start year was Feb. 2004. Our other major funders, (The Greater Milwaukee Foundation, Faye McBeath Foundation, and the Helen Bader Foundation), all provide match funding on a year to year basis. Connecting Caring Communities builds new partnerships and tests new ideas. Even after grant funding ends, its lessons and models will be sustained by the organizations coming forth to work in partnership with others to improve the lives of older adults and their caregivers. |
